After all, root canals are commonly associated with treating painful tooth infections. But what happens when your dentist recommends a root canal even though your tooth doesn’t hurt? It may seem unnecessary to undergo an emergency dental procedure when you’re not experiencing discomfort, but the absence of pain doesn’t necessarily mean your tooth is healthy. Here are several reasons your dentist may recommend a root canal even when you aren’t experiencing symptoms.

The Tooth’s Nerve May Already Be Dead

One surprising reason you might need a root canal without experiencing pain is that the nerve inside your tooth has died. Each tooth contains a soft inner area called the pulp, which houses nerves and blood vessels.

When bacteria enter the pulp through deep decay, cracks, or dental trauma, they can cause inflammation and infection. Eventually, the infection may destroy the nerve tissue. Once the nerve dies, you may stop experiencing the pain that initially accompanied the problem.

Unfortunately, the infection doesn’t necessarily disappear when the nerve dies. Bacteria can continue spreading through the root canals and into the surrounding bone. A root canal removes the infected tissue, cleans the inside of the tooth, and helps prevent further complications.

Dental X-Rays Reveal an Infection

Sometimes, a dental infection develops without obvious symptoms. You may feel perfectly fine while an infection is quietly affecting the tissues surrounding your tooth’s root.

During an emergency dental examination, your dentist may take X-rays that reveal signs of infection, including inflammation or bone loss around the root tip. These findings can indicate that bacteria have spread beyond the tooth’s pulp.

Even without swelling or discomfort, treating the infection promptly is important. A root canal can eliminate the source of infection and help preserve your natural tooth.

You Have a Deep Cavity That Has Reached the Pulp

Not every cavity causes noticeable pain, especially during its early stages. However, when tooth decay progresses deep enough to reach the pulp, a root canal may become necessary.

You might not experience sensitivity or discomfort, but that doesn’t mean the damage is minor. Your dentist will evaluate the extent of decay and determine whether the pulp can recover or whether the infected or irreversibly damaged tissue needs to be removed.

Delaying treatment can allow bacteria to spread, potentially resulting in an abscess or more complicated dental emergency.

Previous Trauma Has Damaged the Tooth

A fall, sports injury, or accident can damage the pulp even when a tooth appears relatively normal. Sometimes, the nerve loses its blood supply following an injury, causing it to die gradually.

You might not notice any immediate symptoms, but your dentist may identify changes in tooth color, unusual findings on X-rays, or other signs of pulp damage during an examination.

A root canal may be recommended to prevent infection and preserve the injured tooth.

If your dentist recommends a root canal, ask about the findings behind that recommendation. Prompt treatment can help prevent an infection from worsening, reduce the risk of an emergency, and potentially save your natural tooth.

About the Author

Dr. Matthew Scarpitti earned his Doctorate of Dental Surgery from Creighton University.
